Report to be generated: Help needed

Hi Everyone..

Need some help from experts.

Sample file attached..

1. There is input list where on daily basis there will be records will be added

2. Every time need to unique values from Input values (code+Branch Name) and paste in output colum J and k, paste formula

3. I don't want to use master data of code, branch as it will be a hudge database and don't need report for all

4. Is there any way (formula/VBA) to find out new entries,sums up from Input Values and append/copy in output format

Kindly help if possible.
