Different listbox, column header sheet in rows in the listbox

How can you make a listbox to show me a different distribution we have seen, ie the headers of the sheet shown in rows to see name information concerning each individual ?? for example as follows:
On the Sheet1

Sending the settings file that has data like this the original. However I have developed two ways. The Mode 1 is with columns together on an event of evaluation. The Mode 2 separate've done only a range of outcomes (Note), another range only Description of evaluation and other Observations
You can choose in the combobox the Mode 1 or Mode 2.
The Mode 2 will show you how the solution you gave me previously.
You can choose which mode is most appropriate to reach the solution.
I put on the form 3 rows of example of how should leave the result in the listbox.

According to the most appropriate way you decide to use, will be the model that I will apply for the remaining sheets are attached

The arrangement of data in SCORE is not the same as your first example and it's no longer worth using the same technique to populate the listbox (I've included in the code a macro Modo1WorkingComboBox1_Change which uses that technique (adapted) which you can rename to a working event handler by removing the part in red (rename the current version to something else too) but I wouldn't recommend using it; it's difficult to maintain/understand.

There is in the attached a more understandable way to populate the listbox, working for both Modo1 and Modo2.

On another matter, but related: The reason it's not straightforward is that your data on sheets Modo1 and Modo2 are arranged in a way for a viewer to understand, then you're asking the machine to work around that (those) arrangement(s) of data.
Now since you seem to be free to re-arrange the data, on sheet3 I have re-arranged the data into a database-like format: 1 row per item of data (a record), with 7 columns (fields). [I've left the macro blah in the Module1 which does this from your data in Modo1.]
Also on sheet3, you'll find a pivot table to play with.
I've left it showing the latest 10 dates (it uses the hidden column P for this) for any name you choose and the average of their Nota. I've also filtered out blank Nota.
I only do this to show you how easy it is now to get a lot of information very quickly. Users/Viewers wouldn't need to see this data as a database, they only need to see sheets with reports generated from this database (the code for which would be relatively simple).
If you want, I could re-write the Combobox1_Change event handler to use the data in this 'database' arrangement

Let me check more carefully, (here are the 20H30 'and I leave my job)
But a brief look, you've managed to use both modes (Mode 1 Leaf, Leaf Mode 2) I just put the two modes to decide your best to achieve the results.
But with excel I see everything I could do. It's great
