Creating a form with attachments

Hello,

I am trying to create an excel file where the end-user has an option to fill the content and attach all the necessary documents and submit.

I was able to get the code to attach a file but the problem with this is that all the files are overlapped (meaning when the 2nd and 3rd files are attached they are getting attached at the same location as the 1st one). I would prefer if they are adjacent to the button and also if the file is displayed as file type Icon rather than just an attachment.

When the user clicks on submit button, I should be able to check if all the documents are attached or not and only if all the documents are attached the user should be able to submit and an email to my email id should be delivered with all the content of the form and its attachments.

I would be greatly obliged if you can help me with this. I have attached the documents for your reference.

Dear Logit,

Thank you for your support, the file that you had provided only attaches the file and sends an email to the list provided in the excel.

My requirement is a little different from that.
1. The "Transfer Form.xlsx" is sent to the end user.
2. The enduser opens this form and fills in all his personal details.
3. The end user then attaches all the documents that are required.
4. When the end user clicks on "Submit" button at the bottom, it should check if all the documents are attached or not and then send the details that he has entered in the form along with the attachments to me.

Será que lo haces mejor en excel de googledrive. allí creas un formulario la persona llena sus datos y adjunta una carpeta con los documentos que requieres

I understand what you say, but I do not write English and on this planet there are not only those who can speak and write in English, the globalized world has solved the communication technically so it is not necessarily able to know another language to be able to communicate with the world whatever language people have.

I wish it was that simple.... my organization has blocked use of google drive for security purpose... so using form and receiving attachments is a far-fetched idea... Can you please suggest any other route ?
 
What version of Excel do you have? If your organization has Office 365, there's Online Survey available using Excel platform.

Alternately, you can utilize SharePoint.
