I will be glad if anyone help me to write the attached equation in an excel spread sheet. The accuracy of the equation will be assessed in the following link:

With Excel 2016 quite hard work:
MDRD:

=175*C2^-1.154*B2^-0.203*IF(D2="Yes",1.212,1)*VLOOKUP(A2,{"Female",0.742;"Male",1},2,0)

CKD-EPI Equation 2021

=142*((C2/IF(A2="Male",0.9,0.7))^VLOOKUP(C2-0.000000001,CHOOSE((A2="Male")+1,{0,-0.241;0.7,-1.2},{0,-0.302;0.9,-1.2}),2))*0.9938^B2*IF(A2="Male",1,1.012)

Could probably be streamlined.
