Please help me use a formula to get data in given sheet

Hi,

I need someone's help to get data in the attached file.
The file has dummy data on Bank Balances and Net worth for a given set of people. I have to extract data using a look-up formula in the block below for same set of people but the trick is that the parts of name have been interchanged,i.e. family name first and given name last.
I am a beginner to excel and most of you may find this a very simple task to do, so please take out some time and help me "become awesome in Excel" starting with this small step before I can take a giant leap.

Hi Himanshu ,

See your file now. I have used helper columns , but if you want you can combine all of the different formulae into one.
