Index match formula problem

Hi All,

Please help me to check if its right to use the index match formula in my data.
I have 5 column data pertaining to the following.
Im trying to key in the category & Bev Cost and see if the Bev Cost % would adjust based on the two category.

I have made some changes to the formula used in column H and copied it down. I'm not sure if this is what you were after; it simply returns the value from column E corresponding to the category and price range.
