Double lookup

Help! I am trying to lookup a two values from one worksheet in another and return a corresponding rate.
I need to compare name from one sheet, find the corresponding name in the second sheet, then compare date in the first sheet to a date range in the second sheet, and populate the corresponding rate for that name and date range back in the first sheet.
Any guidance appreciated.

And regarding your issue, as BBD suggested you consider uploading a sample file (including manual examples of desired output if applicable), it'd be very useful for those who read this and might be able to help you, relieving the contributor of having to build a test file, if necessary. Thank you.
