Creating a Questionnaire in Excel

Hi everyone,

I have a question regarding creating a questionnaire in excel.

The document that I want to create in excel, is currently in Microsoft word format. I have attached it to this document.

I would like guidance on how I can create this questionnaire in excel format.

Any suggestions would be greatly appreciated!

Hi nma, and welcome to the forum! :awesome:

Here's what I would start with. Just did a straight copy of the form, cleaned up the formatting. Put some formulas in the bottom to make the math portion automatic. Now a user just needs to mark a cell in each row, and calculations at bottom will fill in themselves.
